Description: The New York State Kinship Guardianship Assistance and Non-recurring Guardianship Expenses Programs (KinGap) are benefits programs for children move from foster care to guardianship by a relative. This form is the agreement for the KinGap program, which may be submitted to the Office of Children and Family Services after the relative guardian has been certified for six months, a permanency hearing has been held, an abuse and neglect fact-finding has been completed, and an application on OCFS Form 4430 has been submitted.
