Description: Discoverable information in litigation is often contained in social media accounts with companies such as LinkedIn. In connection with a subpoena for such information companies such as LinkedIn may want to obtain a consent and authorization from the account holder to release account information and waive all claims against the company. This form provides a template for a consent and authorization to release LinkedIn account information and a waiver of claims against LinkedIn.
