Description: Under New York law persons receiving or applying for disability benefits and social services are entitled to a fair hearing before an administrative law judge if they have been denied benefits, have had their benefits cut off, and many other reasons. To adjourn or reopen a fair hearing that has been granted a person must submit this form to the Office of Temporary and Disability Assistance (OTDA).
