Description: Under New York law persons receiving or applying for disability benefits and social services are entitled to a fair hearing before an administrative law judge if they have been denied benefits, have had their benefits cut off, and many other reasons. New York law also allows an appellant in a fair hearing to request the evidence that the City will use at the fair hearing by submitting the form included here.
